My own answers would be:
1: Frankly, it would be a certain spruce tree in a wood next to our house. I was frightened by it when I was a little girl, and it still makes me very uneasy. I know it is sick, since I know it is not very, if at all, dangerous, but I want to be honest with you. Please don't laugh!
2: Maybe I would transform it into one of those little christmas trees you have on tables covered in glitter and other decoration, singing a silly song while doing an even sillier dance. He he...
3: Probably one of those times I was bullied or the time this May when I happened to get a plastic bead deep into my left ear. It was so terrible and so embaressing since I was almost nineteen years old. Or once when I was four and I tripped over in a hill in a forrest and a sharply cut piece of wood stung into my neck and had to go to hospital to get sewn. Or once when I have had a fight with someone. It's hard to say.
4: Lying on my bed reading a good book while listening to as good music... Or something really funny, I mean laughable.
5: I would like it to be silver cat who just clawed the dementors away.

I really like this one because it made me really think.

1. boggart shape? Probably (like Molly Weasley) someone in my family lying dead in front of me.

There was this one time a guy broke into our house, and he came in through my room... and he tried to suffocate me with a wet towel, and he slapped me, and tried to push my chest of drawers over on my bed... it was just terrible, I really hate talking about it... luckily our security system buzzed and the police got hi. It can be annoying when thread titles are changed. And everyone hates when they get a great thread idea, they even search for duplicates, and then someone finds the same topic that they tried to but didn't, and their great idea is close because it has already been talked about. But look at it from the other point of view, Furienna. The profs do an awesome job. They have so much to do, from helping newbies out to getting rid of terrible threads to just plain and simple keeping CoS up and running, that it would be impossible for them to be perfect. No one is flawless, and they put so much time into this just to help everyone else. That's something that most people don't appreciate. So some things they do can be annoying - I know first hand, having thread titles changed seemingly without a reason and therefore no one knowing which thread was which to getting entire threads deleted when I thought they shouldn't have been - but ultimately they are doing an amazing job. Would you do better, Furienna? I know I wouldn't.

I'd probably make it be paralysed or something.

errrmm.. I probably wouldn't be able to think of one and so I would get stomped.

A hare- you may think they're pathetic, but they can give you a serious slashing with those back paws! And anyway, hares are reputed to be magical creatures.

When I almost drowned in a pool because I didnt know how to swim (I know how to swim now by the way)

Honestly...I don't know. Most of the phobias I have are contingent--that is, they wouldn't particularly scare me in a crowded classroom. Maybe a clown. Clowns are scary.

Hmm... turn it into a vicar with polka-dotted clothing.

A particularly unpleasant period of my life during which my then-boyfriend was constantly threatening suicide, among other things.
